It’s easy to take advantage of your EyeMed program. Simply follow the steps below:

In-Network Benefits

1. Locate the Nearest EyeMed Provider: Members can locate providers by:
   a. Calling our Customer Care Center to use our automated system or to speak with a representative;
   b. Referring to their member brochure;
   c. Logging in to access the online provider locator.

2. Schedule an Appointment: Many in-network provider locations have evening and weekend hours; some locations even have walk-in appointments available to accommodate patients' busy schedules. Members should identify themselves as having EyeMed benefits when making an appointment (though an ID card is not required).

3. Receive Services: Members should present their ID card at the time of service to help expedite the process. (If you don't have one, the provider will verify your eligibility for you. You can also log in to EyeMed's secure member site to request a new ID card.) Once eligibility is verified, members may access their benefits and are only responsible for paying the designated plan co-pays and any other incurred charges. The provider arranges eyewear fabrication and delivery.
   

Out-of-Network Benefits

1. Request an Out-of-Network Claim Form: Download an out-of-network claim form (.pdf) or call the Customer Care Center to have one mailed to you within 24 hours. Forms can also be emailed or faxed. Some plans have custom out-of-network claim forms, which can be accessed through our secured member site.

2. Schedule an Appointment: Make an appointment with the out-of-network provider of your choice.

3. Pay for all Services: Pay for all services at the point of care and ask the provider for an itemized receipt.

4. Submit Out-of-Network Claim Form: Fill out and submit the out-of-network claim form with paid receipts to us for processing. Out-of-network reimbursements are sent directly to the subscriber weekly or biweekly depending upon the plan type. Payment will include an EOB.
