Don’t see your preferred eye doctor? Download and complete the Provider Referral Form (.pdf) and forward it to our Provider Relations Department using our online contact form. We will contact the provider about joining our panel. All doctors must accept our terms and conditions and complete our credentialing process before servicing EyeMed members.
