Q. How long has EyeMed been in business?

A.  EyeMed has nearly 20 years of vision benefits experience, and we're part of Luxottica Group, a leader in the global optical industry since 1946.



Q. How do I obtain contact information for my Account Service Manager?

A. Please call us at 888-362-7463 if you are an existing client and you are not sure who your Account Service Manager is.




Q. How do I obtain a login and password to access the secure Benefit Administrator pages?

A. EyeMed will set up an account to be managed by one person within your organization. That person, designated as the Account Owner, should contact the account representative to register. The Account Owner will receive a login and password, and can then register and maintain additional users on that account. Any questions regarding registration should be directed to your Account Service Manager.

Q. What kind of plans do you offer?

A. EyeMed offers a variety of plans to meet diverse employer group needs, including fixed fee (through outside underwriters) and fee-for-service funded benefits and discount vision programs. Please visit our Flexible Benefit Plans page to learn more.

 





Q. Do you offer products in all states?

A.  Yes, EyeMed's plans are available for companies located in all 50 states and the District of Columbia. We can also provide benefits for employees residing in Puerto Rico and the U.S. Virgin Islands.




Q. What is your minimum group size?

A. EyeMed requires a minimum of 10 employees enrolled in a program.

 





Q. How big is your network?

A. EyeMed's network offers access to up to 40,000 providers at 20,000 locations nationwide. Depending upon the product selected, your plan may be supported by a sub-set of these providers. Regardless of the panel chosen, your employees will find thousands of private practice doctors as well as the nation's leading optical retail brands to satisfy their vision care and eyewear needs. Q. Will my employees have to file claims for benefits?

A. No. Employees do not file claims for in-network benefits. If your company offers out-of-network benefits and an employee decides to use the benefit at a non-participating provider, the employee must complete and submit an out-of-network claim form in order to be reimbursed for the services and materials.




Q. What customer service options do you have available for my employees?

A. EyeMed offers the most extensive customer service hours in the industry. Representatives are available to respond to member calls from 8 a.m. to 11 p.m. ET Monday through Saturday and from 11 a.m. to 8 p.m. ET Sunday. We also offer self-service options through this Web site and via our toll-free phone line 866-9EYEMED.
