EyeMed provides vision benefits through some of the nation’s largest health, dental and life insurance carriers. Patients may present ID cards or materials for vision plans administered by EyeMed under other company names.

These products should be treated as any other EyeMed plan, and benefits can still be processed through our Online Claims Management System. Please contact us with any questions regarding our private label relationships or plans.
