Provider Referrals Welcome
If your employees are looking for a provider not currently on the network, download and complete a Provider Nomination Form (.pdf) and forward it to our Provider Relations Department using our email form. We will contact the provider about joining our panel. All doctors must accept our terms and conditions and complete our credentialing process before servicing EyeMed members.
